Please don't send health details through this website
Our contact form sends your note securely to our practice, but it is meant for getting in touch, not for medical information, and email is not secure. Please do not include diagnoses, symptoms, medications or test results. For emergencies call 911. Once you are a patient, you can share health information securely through our patient portal.
What we collect
- What you type in the “Stay in the loop” form: name, email, phone (optional), message (optional), and how you heard about us (optional)
- If you book an intro call: what you enter on our scheduling page
How we use it
To reply to you, to tell you about our opening date, waitlist and updates, and to understand which outreach reaches people.
Who else touches your information
Your inquiry, any intro-call booking and the patient portal are handled by the secure records system our practice uses.
Your choices
You can unsubscribe from emails at any time, and ask us to see, correct or delete what you sent through this site by writing to hello@redwoodofm.com.
Text messages
If you agree to receive text messages from us, we use your mobile phone number and your consent only to send you the texts you agreed to and to provide our text messaging service. You can reply STOP to any of our texts to opt out.
Mobile phone numbers and text messaging opt-in consent are not shared with or sold to third parties or affiliates for marketing or promotional purposes. The service providers that deliver our texts handle this information only to send them.
